Default Element Catch Can Install on my Built Motor

Hello all.
I have a Cusco catch can that doesn't really catch much at all.
I've decided to go with the Element Catch can to help alleviate my blow by woes.
Phil has patiently told me how to install the can two or three times now and I just haven't gotten around to it. Well I'm going to VIR this weekend and would like to have it in for the track.
I have the can mounted in the cruise control location, right beneath my strut bar. I don't have any lines hooked up.
Here are the applicable parts of my setup for a discussion:
V8 Spec C heads that have twin breather ports on each valve cover.
EJ257 case
02 WRX intake manifold.

(1) If I have it right, Phil suggests that the two forward (driver and passenger)valve cover breathers are teed to the -10AN on the can.
(2) Then the crankcase breather, which (to my understanding) is the breather hose coming off the block about half way between the center line of the block and the passenger valve cover, is run to the -12AN on the can. Do I need to tee this in with any other line?
(3) Then the two rearward valve cover breathers are connected together and teed to the breather right in the dead center of the block underneath the throttle body. These hoses aren't going to the can in any way right?

Please provide me any tips you can think of.
