Default Bizarre USB problem(s)

I have a PIII running Windows XP Home SP2 (this is my alternate machine, not my everyday computer) with some very strange USB problems. It has two front panel USB headers and two USB ports in the back. Two main problems:

1. The front panel USB headers don't seem to work at all, even though the internal cable is connected to the appropriate motherboard connector. I'm not 100% that it's connected correctly, however, as the motherboard documentation doesn't actually list the pinout assignments. The connector is 2x5; the motherboard has 10 pins, so I'm not sure which end of the motherboard connector is the +5V and which is the S-GND. I'm worried that connecting it the wrong way will permanently damage either the USB device or the motherboard.

2. The back panel USB ports work - kind of. For example, if I plug an 802.11b wireless adapter in, it is recognized and works fine - but it won't hot-swap. I.e., if I remove it, then plug it back in, it won't be recognized. If I reboot with it plugged it, in works fine again. The other USB port doesn't seem to work at all.

3. If I plug any kind of hub into the "working" rear USB port, Windows recognizes that a device is plugged in, but then says that the "Device is not working properly". In device manager, the hub is listed as an Unknown device.

4. Any other USB devices (camera, scanner) aren't recognized at all by any port.

I've tried uninstalling the USB Root Hub and controller with no luck. I don't recall the controller model off-hand (I don't have access to the computer at the moment), but it's an Intel USB chipset.

Is my USB controller totally hosed?
