Have a Nice Day? An employee just came to my office and asked me if she was having a heart attack

CN at the bottom.



A ___ y/o female patient was laughing nervously upon her arrival at my office. She appeared to be alert and oriented x4, and had normal skin color. Her skin appeared to be dry from where I was sitting. The Patient asked me if she was having a heart attack. The pt did appear to be mildly anxious. I asked what her symptoms were, and she stated that she felt a little light headed, and she had pain in her left shoulder. The pain was described as a dull pain, and does not radiate down left arm, to back, or to the chest. However, the pt also complains of left wrist pain. Signs and symptoms of a myocardial infarction, both typical and atypical, as well as MI case examples are discussed with the pt. the patient denied all symptoms discussed, and stated that she probably wasn't having a heart attack. More MI examples were explained to the pt, including my fathers recent episode in which it was theorized that he had a mild heart attack 5 days prior to calling 911, and the pt still believed she was not having an MI.

Patient went back to her cube, but reappeared at cubes outside my office 4 or 5 minutes later. I asked some follow-up questions and performed a focused physical exam of the left shoulder. Upon palpation, pt states that shoulder pain "hurts, but in a good way; like you should work it more." By "work it more," pt is referring to massaging. Pt states she has a history of PVCs, but says PVCs are not occurring now, nor have they been occurring today. Pt did take two 325mg aspirin about one half hour ago. As of now, the aspirin are not providing any relief of shoulder or wrist pain.

Pulse is 80, strong and regular. Skin is warm and dry. BP 146/90. Pt states that a BP taken last week was 130/81, and that her normal BP is 120/70. No other vitals taken.

Pt also states that she operated a snow blower for some time prior to coming into work today. When asked when the arm pain started, pt states that she has been feeling it for a few hours prior to visiting me. I advised the pt that if her condition were to change at all, pain change, move, get worse, light-headedness return, etc., that she is to call me. Patient returned to her cubicle without incident.



CN: The fact that people here at work know I'm an EMT sometimes makes my day more fulfilling and rewarding, even though I get less 'real work' done.

SO... her arm hurts from snowblowing and she thinks she is having a heart attack. Typical overcomplicated attention whore looking for the big strong fireman type to rub her shoulders to make up for a thankless relationship at home. FYI - If you want you can bone her tonight.

The fact that you work with a self-indulgent creampuff who gets painful fatigue from operating a snowblower, and is so sedentary that she doesn't recognize exercise-induced soreness makes your day "more fulfilling and rewarding?"

I really wish I would set the bar that low. I would be so much happier.

I work in the quote/contract dept of a pre-hospital medical equipment distributor. I wish her anomaly was photoshopped.

She has a cardiac history, as does her husband, and IIRC she lost one of her parents to a heart attack within the last couple years. Overweight, yes, sedentary, I don't think so. She can be 'moody,'; and reports are that she is on meds for that. She is very much not an attention whore around here.

Sure, try setting your bar low and see how things go for you. Me, I currently am not a practicing EMT in the field, but I'm still licensed and will excercise my chops when a sitaution presents itself and applying EMT skeelz are appropriate. No matter how big or small the situation is, I get satisfaction out of helping and/or making a difference.

Typical emt/paramedic: Use big medical words to impress joe schmo. Granted I am thankful emts/paramedics help people on a daily basis, most act like "who's the man?" Do yourself a favor, if you think someone's having a heart attack or any other disease you might "diagnose" to be harmful to a person, ask them if you could call 911.

most women experience a harsh pain in the jaw when they have heart attacks which is why they are misdiagnosed and why more women die from heart attacks than men. They rarely have pain in their chests.

Sometimes I really wonder about people like her. I know some people have a very high pain tolerance and that men and women present differently with MI, but if it worried her enough to even ask she should have seen a doctor ASAP (and not driven herself there). I can see not wanting to 'waste' an ambulance trip given the circumstances though.

I was talking to a fast food manager a while back and she said the assistant manager called her one night. The assistant was working a regular shift and thought the store might be on fire so she called the manager to see what she should do.
