Lack of lubrication or contaminated lubrication. And yes, unfortunately that head is junk.

Luckily it is the driver side head. You can use USDM 2.5RS or Outback DOHC head and swap the valves and valvetrain as casting is the same. Not sure if that 20K cam can be salvaged though.

As said, the castings are the same you just have to swap the 20K valves, springs and retainers and re-adjust valve clearances! You can get underbucket shims from Japanpart.com or you could even try if the USDM EZ30 underbucket shims will fit!

i have a half set of jdm v4 sti valves, if you need them. i had my heads rebuilt and they found 8 bad valves, i couldnt afford the jdm valves so i put machined supertech valves in instead.
the jdm valves are $94/ea for the intake and $131 for the exhaust from grahamgooderacing or $455/intake and $497/exhaust from japanparts.

pm subiking here on nasioc, he was the one that dealt with the head shop where my heads were rebuilt, he has all the info would about the vavles that were used. i do know they were supertech stainless steel udsm sti valves that were machined to the proper length for the 20k heads.

i started the engine that those heads are on now for the first time last night and they seem to be good...

Give Chris at EngineLogics (Houston,TX) a call. I've seen what he did to a Honda head that had a very similar issue - great work. Line bore (if needed) and repair the head. Of course it might be a cheaper if you have a line on a spare K head.
