Default Injector fail? Please help.

OK, to make this quick, recently converted to 1200cc injectors for e85 tune. Car ran great for a week after tune, little rough on warm starts. I fixed a fuel leak Mon then Tues night finally had to turn over engine several times to get start and it immediately runs horrible. Misfire codes, backfiring, horrible hesitation, running super rich-gas on hand from exhaust. Pulled the injectors and found one had lost its end cap, black rubber o ring, and 2 plastic o rings. The tip on this one had a black residue whereas the other 3 were fine. All parts were recovered from the fuel rails except the end cap! It might be lost in the fuel system. We replaced these with my 750cc injs, car started fine then same problem. My tuners have been a great help and we have tried many corrections and it appears to be a mechanical issue. Possible bad cylinder, MAF to turbo vacuum leak? Any suggestions would help, thanks to all so much.
mods- Perrin V2 fuel rails, Deatch 750s, Walbro, Bloush 18g, tbe, ngk one step colder plugs, Phatbotti tuned

Maybey that end cap that you didnt recover got in somewhere its not supposed to be and is fng stuff up somehow?

That is my main concern, thought those went straight to cyl and would incinerate (its plastic). Unless the cap is stuck in the line (not visibly), maybe could poke it through, idk. I'll read up on clogged fuel rails and see if symptoms match. Thanks for the help, any other ideas?

What brand are the 1200's?

I would run a compression test and maybe a leakdown. But it is pretty important to find where that endcap went. Maybe you need to pull the intake manifold and look around.

Juan and I did leak test, compression test. No major leaks, compression ok across the board. Replaced spark plugs and car seems to be running much better today, fingers crossed.

Car is running, huge thanks to Juan, Ron and Mikey. I will reserve judgement on the injectors until I get them reinstalled.
