You will need:

Clutch Disc
Pressure Plate
Pilot Bearing (In Flywheel)
Throwout Bearing

You may need a flywheel depending on the condition of yours. You can buy aftermarket clutch kits that come with everything you need and some even include the flywheel. Some people change the dust boots and some other minor extras but most of the time it is not necessary.

You may want to check out the sticky at the top of the forum page. There should be a transmission and clutch FAQ to answer any questions that you may have.
