It will not work with the twinscroll manifold. It is designed for the 02-05 WRX.

Yes it does. It comes with a aluminum Y-Pipe, new silicone hoses for the y-pipe, and hose clamps.

2) Will the Cusco Turbo Heatshield fit with a larger tmic?

The Perrin will fit on the VF34 and the Cusco Heat Shield will fit on a larger TMIC.
