Yes, same grommet. Pull it from the OEM tank, swap it over and you're good.

No. Without knowing what radiator/bung size you have that would be a guess. A couple $ and your local auto parts or hardware store and you're good.
The reservoir is thread 1/8NPT for whatever barb adapter size you need to use.

i bought a koyo radiator but it hasnt come in the mail yet. you wouldnt happen to know what i would need off the top of your head would you? i'm just trying to minimize the trips to the auto store when i finally put everything in ya know?

Sorry of the super delayed response on this.
You'll need an 1/8NPT fitting with a barb on it to match the barb on the radiator. Then a length of hose (that fits the above barbs) long enough to run from radiator to reservoir. I like to run the line under the intake manifild and across the back of the engine.
