Default what causes the tranny to fail?

just wondering did anyone blew his tranny before?
What caused it to break?
was it because you shifted up or shifted down during high speed?
or did the tranny just broke when the car was in gear?

All it takes is a bit of hard shifting into the next gear up to cause some stress fractures on teeth... then as soon as you shift easy, BOOM! You ain't goin NOWHERE! :-/
