Default Forester XT Clutch Trouble

I've got a question regarding my 04 FXT's clutch. First off, it's starting to let up very high in its stroke. It now chatters a good bit out of first from a stop. Recently, its begun to jump RPMs if 4th and 5th only on the highway. But it wont do it in 4th or 5th at low RPMs. I thought it would be more prone to slipping at lower rpms in a higher gear. I'll be at 80 in 5th and punch it, then it jumps 1,000 rpms very suddenly. Is this still an indication of the clutch beginning to slip?

Yeah, mine only lasted to about 110k miles. Mainly from burning it up driving in deep sand on the beach for over 10 years.
