Question T/O Bearing + Wear on input shaft collar (T/O bearing surface)?

Question 1 - New throw-out bearing for my ACT street disk is not centered within the outer race/collar. The old one wasn't either. Is this a problem or intentional?

Question 1A - T/O bearing retainer clips (to fork)- really neccessary? The shop that did my 1st ACT install (@~80K) didn't re-install them. ..which might have caused Q#2?

Question 2 - The shaft/sleeve (surrounding input shaft) that the T/O bearing rides on has some low spots (12 to 4+ region) and has a step where the T/O bearing "parks". Additionally there is a step in the face of the collar. Problem or non-issue?

Clutch works (worked?) smoothly with above and tranny has 155K and with the exception of 3->4 shift above 4K (shift slowly or it grinds slightly), seems to be in good shape. (I have put 151K of the 155K on the car.)
